Traditional Thai massage is not a massage in the Western sense. There is no kneading or oil used, and you don't lie on a massage table. Instead, the therapist works with thumbs, palms, elbows, and feet along specific lines on the clothed body, accompanied by guided stretches. Here's how a session works, what to wear, how long it lasts, and for whom it is not suitable.

What Nuad Thai means

In Thai, it is called Nuad Thai. It is part of traditional Thai medicine and has been passed down for centuries, mostly in temples and from hand to hand. On December 12, 2019, UNESCO added Nuad Thai to the Representative List of the Intangible Cultural Heritage of Humanity. It is the only massage form with this entry to date.

The idea behind it: The body is traversed by lines, the so-called Sen. If something is stagnant there, discomfort arises. The treatment works along these lines and is intended to restore the flow. This is a cultural explanatory model and not an anatomical description. You don't have to believe it to benefit from the treatment.

How a session works

The work is done on a firm mat on the floor, not on a massage table. This is not a minor detail, but a prerequisite: only in this way can the therapist use her own body weight and guide you into positions that are not possible on a narrow table.

The process almost always follows the same sequence, from bottom to top:

  • Feet and legs. The beginning, usually in a supine position. Here the pressure is built up and you get used to the intensity.
  • Hips and lower back. Often the area where the first stretches occur.
  • Back and shoulders. Often in a side or prone position, sometimes seated.
  • Arms, hands, neck and head. The end, usually calmer than the rest.

In between, the therapist guides you into stretched positions. Some of them resemble yoga, which is why Thai massage is sometimes called passive yoga. You don't do anything yourself, you let it happen. This is precisely the most unfamiliar part for many.

What distinguishes it from a classic massage

  • Clothed instead of unclothed. You keep light clothing on; the studio usually provides something.
  • No oil. It is pressed and stretched, not stroked. Oil would get in the way.
  • Pressure instead of kneading. The therapist works focally and holds the pressure, instead of working through the muscles.
  • Stretching is part of it. This is the biggest difference and the reason why the treatment feels different afterwards than a classic massage.
  • Mat instead of table. More freedom of movement for both sides.

We have compared this in detail here: Thai massage and normal massage compared.

Duration and clothing

A complete treatment usually lasts 60 to 120 minutes. Less than an hour leaves too little time for the full process from head to toe, so shorter appointments are usually limited to one area, such as the back and neck or foot reflex zones.

Wear something in which you can sit and bend your legs: loose trousers, a T-shirt, no belts, no zips on the back. Jewellery and watches stay outside. Do not eat immediately beforehand, as the stretches also work on the abdominal area.

How intense it is

Significantly more intense than a wellness massage, and that is intentional. A comfortable pressure is one where you can continue to breathe calmly. As soon as you hold your breath or resist, it is too much. Say so then, good therapists will ask anyway. Muscle soreness the next day is possible and not a bad sign.

More on this in two separate posts: How painful is Thai massage and Why does Thai massage hurt.

When it is not suitable

Thai massage works with pressure and traction on joints. In these cases, it should be medically cleared beforehand or avoided:

  • Pregnancy, except with therapists specifically trained for it
  • Fresh injuries, fractures, slipped discs
  • Osteoporosis and other diseases affecting bone density
  • Thrombosis or suspected thrombosis
  • Acute inflammation, fever, open skin areas
  • After operations, as long as wound healing is in progress

When in doubt, the same applies as everywhere: ask first, then book.

What can be done at home

No device can replace the stretches of a Thai massage; it requires two people. What devices can take over is the other part: the sustained pressure along the back. Massage chairs with stretch programs lower the leg rest and recline the upper body, gently stretching the spine while the rollers work. This is the gentle relative, not a replacement.

If you want to try it out, it's best to sit in one: overview of all massage chairs, free test sitting at over 40 locations. How a full Thai body treatment works in detail is described here: Process of a Thai full body massage.

Summary

  • Traditional Thai massage is called Nuad Thai and has been on the UNESCO Intangible Cultural Heritage list since December 2019.
  • It takes place clothed, without oil, on a mat on the floor.
  • It involves sustained pressure along the Sen lines and guided stretches.
  • A complete session lasts 60 to 120 minutes, from head to toe.
  • For pregnancy, osteoporosis, thrombosis, and fresh injuries, consult a doctor beforehand.
