Pain after a massage: normal or not?

In short

You leave the studio relaxed and wake up the next morning with a back that feels as if you had been weight training. Or with a headache that was not there before. That is unsettling. After all, a massage is supposed to do the opposite. Usually it is harmless and over within one to two days. Sometimes it is a signal you should not ignore. This article shows you how to tell the two apart.

Summary/TLDR

A dull ache similar to muscle soreness for one to two days after an intense massage is normal, especially after the first treatment or after a deep tissue or Thai massage. A mild headache, tiredness and feeling thirsty on the same evening are also normal. Not normal are: stabbing or radiating pain, numbness or tingling in the arms or legs, pain that increases beyond three days, pronounced bruising, swelling, fever, or pain in the calf after a leg massage. These signs need to be checked by a doctor.

Why a massage can leave you sore at all

A firm massage puts mechanical load on the tissue. That is not a side effect but the mechanism. Four processes explain why you feel something afterwards:

  • Micro-irritation of the muscles. Sustained pressure on hardened fibres creates minimal irritation, similar to unaccustomed exercise. The result feels like muscle soreness and is in principle the same thing.
  • Changed muscle tone. When a permanently tense muscle suddenly relaxes, the balance of the body changes. Neighbouring muscle groups have to redistribute the work for a short time, which can ache for one to two days.
  • A surge in blood flow. The increased blood flow carries away metabolic waste, but can temporarily create a feeling of pressure or tension.
  • Irritation of connective tissue and fascia. Especially with techniques that scrape or pull over a wide area, the connective tissue reacts more sensitively than the muscle itself.

After the first massage in a long time, the reaction is expected to be strongest. If you go regularly, you will hardly feel it any more. The muscles are simply used to the load.

Back pain after a massage

The most common case, and also the one that worries people most. To assess it, the type of pain helps more than its strength.

Feature Harmless Have it checked
Character dull, aching, over a wide area stabbing, sharp, at one point
Course strongest on the second day, then decreasing increases over several days
Duration up to about 72 hours unchanged beyond three days
Radiation stays local runs into the leg, the arm, the ribs
Movement improves with movement gets worse with movement
Accompanying signs none numbness, tingling, loss of strength, fever

The most important point in this table is radiation. Pain that runs from the back into the leg or comes with numbness has nothing to do with muscle irritation and needs to be assessed by a doctor, regardless of how the massage went.

If the symptoms appeared after using a device rather than after a treatment by a person, the causes are partly different. We cover this in Is back pain after using a massage chair normal and Is pain after using a massage chair normal.

Headache after a massage

This happens too, especially after intensive work on the neck and shoulders. The usual explanations:

  • Lack of fluids. By far the most common cause. Many people go into the appointment under-hydrated anyway, and the increased blood flow makes this noticeable. One or two glasses of water afterwards often solve it completely.
  • Released neck tension. When very tense neck muscles relax, your head posture changes. A tension headache for a few hours is a common side effect.
  • Fluctuating blood pressure. Getting up too quickly after lying down leads to circulatory reactions. Take the two minutes of rest the studio offers.
  • Too much pressure on the neck. The neck tolerates less than the buttocks or thighs. Speak up sooner next time.
  • Fragrances. Essential oils and room scents reliably trigger headaches in some people. This is easy to test by asking for a neutral oil at your next appointment.

A separate case is migraine. In people with migraine, a massage can occasionally act as a trigger, while in others it helps prevent attacks. This varies from person to person and is covered in detail in Can massage trigger migraines.

Warning signs where you should not wait for a headache to pass: a sudden, very severe headache ("the worst ever"), vision problems, speech problems, weakness on one side, a stiff neck with fever. That is an emergency and, in all likelihood, has nothing to do with the massage.

After Thai massage and deep tissue, more is normal

With these styles, a stronger after-reaction is to be expected. Thai massage works with body weight, elbows, knees and wide stretches. Deep tissue deliberately goes into deeper muscle layers. One to three days of muscle soreness is the rule here, not the exception.

Here, too, the limit is the same: dull and widespread is fine, stabbing and radiating is not. If you have pain in a joint rather than in the muscle after a Thai massage, a stretch went too far. Mention it at your next appointment and have it checked if the symptoms persist.

Why this intensity is part of the concept at all is explained in How painful is Thai massage and Why does Thai massage hurt.

What helps with the symptoms

  1. Drink. One to two litres spread over the rest of the day. The simplest and most effective point.
  2. Warmth rather than cold. For symptoms like muscle soreness, warmth helps: a warm shower, a hot water bottle, a bath. Cold is only advisable for swelling or bruising.
  3. Light movement. Walking, gentle stretching. Do not rest completely, but do not train either.
  4. No intense sport on the same day or the next. The muscles are already irritated.
  5. No alcohol that evening. It increases the lack of fluids and with it the tendency to headaches.
  6. Get enough sleep. The symptoms fade fastest during sleep.

On the frequently asked question about showering: there is nothing against a warm shower afterwards, but it should not be very hot or ice cold. The details are in How long not to shower after a massage.

How to avoid it next time

  • Comment on the pressure while it is happening, not afterwards. A massage should work at the boundary between firm and uncomfortable, not beyond it.
  • Mention pre-existing conditions and medication in the initial consultation, especially blood thinners, osteoporosis and slipped discs.
  • Drink enough before and after.
  • Start gently. At your first appointment after a long break, choose a gentler style. Which ones are suitable is explained in Which massage for beginners.
  • Go more regularly rather than less often and harder. How often makes sense is covered in How often to have a massage for severe tension.

When you should get medical advice

With these signs, do not wait:

  • pain that radiates into the leg or arm
  • numbness, tingling or loss of strength in the arms or legs
  • pain that does not ease after three days or increases
  • large bruises or swelling
  • fever after the treatment
  • pain, swelling or warmth in a calf after a leg massage: thrombosis needs to be ruled out here, and promptly
  • chest pain, shortness of breath or a racing heart
  • a sudden, extremely severe headache, vision or speech problems

This list is not meant to frighten you. The vast majority of after-effects are harmless. It is there so that you recognise the exceptions instead of dismissing them as "normal muscle soreness".

Conclusion

One to two days of dull aching after a firm massage is to be expected and usually a sign that real work was done. Drinking, warmth and light movement take care of the rest.

The line is not drawn by how strong the pain is, but by what kind it is: dull and widespread is normal, stabbing and radiating is not. And if you say sooner at your next appointment that it is getting too much, you have solved the problem at the root.

Frequently asked questions (FAQs)

How long can pain last after a massage?

Symptoms like muscle soreness usually peak on the second day and fade by about 72 hours after the treatment. If they last longer unchanged or increase, have the cause checked by a doctor.

Is muscle soreness after a massage normal?

Yes. Firm massage irritates the muscles mechanically, similar to unaccustomed exercise. The reaction is strongest after the first treatment in a long time and after Thai or deep tissue massage, and it decreases noticeably with regular appointments.

Why do I have a headache after a massage?

Most often because of a lack of fluids. Other reasons are a changed head posture after neck tension has been released, circulatory reactions from getting up too quickly, too much pressure on the neck, or a reaction to essential oils. Drinking enough and staying lying down for a few minutes solves most cases.

Can a massage make a slipped disc worse?

With an acute slipped disc, firm massage in the affected area is not advisable. Radiating pain, numbness or loss of strength after a treatment always need to be checked by a doctor. Mention any existing diagnoses in the initial consultation so that the technique can be adapted.

What to do about bruises after a massage?

Occasional small bruises can occur with very intense techniques and disappear within one to two weeks. Cooling helps in the first few hours. Large or numerous bruises, on the other hand, indicate too much pressure or a clotting disorder. Especially if you take blood-thinning medication, discuss this with your doctor.

Should you exercise after a massage?

Better to avoid intense training on the same day and the next. The muscles are already mechanically irritated, and the risk of injury is briefly increased. Walking, easy cycling or gentle stretching, on the other hand, are useful and speed up recovery.
