A back massager works alongside the spine, not directly on it. The bony ridge in the middle of the back has no muscle padding to absorb pressure. The work is done on the two muscle strands to its left and right, about two to three finger widths away from the center.

What lies in the middle

The palpable bumps along the middle of the back are the spinous processes of the vertebrae. They protrude backward and, in most people, are covered only by skin and a thin layer of connective tissue. Beneath them runs the spinal canal with the spinal cord.

Direct, repeated pressure on this ridge is undesirable for two reasons. It hurts because there's nothing to cushion it. And it accomplishes nothing because the tension is not in the bone, but in the muscles next to it.

Where the work actually takes place

To the left and right of the spinous processes runs the erector spinae, a long muscle strand from the pelvis to the neck. It keeps the upper body upright and is constantly under slight tension during office work. This is precisely where the massage heads are applied.

Therefore, good devices have an adjustable distance between the nodes. A narrow back requires a narrower setting than a wide one. If the distance is fixed and doesn't fit, the heads will run over the spinous processes instead of alongside them.

How to tell if the setting is wrong

  • Punctual, stabbing pain in the center instead of a dull pressure to the side.
  • A clicking or rattling sound as the massage heads jump over the bony ridge.
  • Pressure marks the next day that are exactly in a line.

If any of these occur, adjust the distance or change your sitting position. A rolled-up towel under the lower back is often enough to shift the path.

The lower back needs more attention

In the lumbar region, the spine is curved forward. A straight massage path will hit the highest points of the curvature particularly hard, while the muscles in the depression remain untouched. Those who focus on the lower back are better served by a more adaptable design.

Which devices are suitable can be found in the article Lower Back Massager. Adjustable models for the entire back are listed under Back Massagers.

When not to use the device

For known spinal changes, a different rule applies than for simple tension. Osteoporosis, severe scoliosis, spondylolisthesis, or post-spinal surgery conditions should be medically evaluated before using a device.

The same applies to pain radiating into the arms or legs, numbness, and loss of strength. These are signs of nerve involvement, and they should be clarified rather than massaged. A device relieves muscle tension; it does not replace a diagnosis.

What a towel can do

The simplest way to correct the path is with a rolled-up towel. Placed between the device and your back, it raises the contact surface and takes pressure off the spinous processes. For narrow backs, this is often the most effective measure of all.

Two towels, placed lengthwise to the left and right of the spine, are even more targeted: they leave the center free and direct pressure to the muscle strands. This costs nothing and replaces the missing adjustment of the node width in many simple devices.

In case of a herniated disc

If there is a disc problem, special rules apply, starting with a medical evaluation. What is possible afterward and how to recognize when to stop is explained in the article Back Massager for Herniated Disc.
