In the case of an acute herniated disc, a massage device should not be applied to the back as long as the symptoms have not been medically assessed. In the phase thereafter, massage can be beneficial for accompanying muscle tension, but it does not affect the disc itself. Knowing this distinction is half the answer.

What actually happens in the event of a prolapse

Between two vertebrae lies the intervertebral disc, acting as a buffer. It consists of a firm fibrous ring and a soft core. If the ring tears, core tissue protrudes and can press on a nerve root. This very pressure generates the typical symptoms.

The intervertebral disc is deep, protected by the spine and back muscles. No massage device can reach it, and none can push protruding tissue back. What a device does reach is the musculature above it.

Why the musculature still plays a role

Around a painful area, the body tenses the muscles to immobilize it. This protective tension is useful, but it becomes a problem itself if it persists for weeks. It restricts movement and causes its own pain.

This is where massage comes in, which is why it is a common component in the rehabilitation phase. It treats the consequence, not the cause. Those who expect this will not be disappointed, and those who expect more will be.

When the device is to be avoided

  • During an acute episode, as long as the pain is severe and the cause is not clarified.
  • In case of numbness in the leg, foot, or buttocks.
  • In case of loss of strength, for example, if the foot drags when walking.
  • In case of bladder or bowel dysfunction. This is an emergency and requires immediate medical attention, not massage.
  • After an operation, until the treating practice expressly approves.

How to proceed after clearance

How to recognize when to stop

Stop immediately if the pain increases during application, radiates into the leg, or if tingling occurs that was not present before. This also applies if the massage itself is pleasant. A stimulus that reaches a nerve feels different from muscle work.

Likewise, a reason to stop is pain that is stronger the next day than before. A slight muscle soreness after the first application is normal; an increase in the original symptoms is not. When in doubt, the same sequence applies as at the beginning: first clarify, then decide.

Sitting is the actual challenge

More load acts on the intervertebral discs when sitting than when standing, especially with a bent upper body. Anyone who spends their workday at a desk changes more with their sitting posture than with any device.

Practically speaking, this means: an upright backrest instead of a hollow back or rounded back, feet flat on the floor, and standing up once an hour. A massage device can make these breaks more pleasant, but it does not replace them. Those who combine both will achieve a greater effect than with one alone.
