Shiatsu massage: side effects and when it is risky

In short

Shiatsu is considered gentle, and usually it is. Even so, many people report tiredness, muscle soreness or a brief low mood after their first treatment, and wonder whether that is part of it. Usually, yes. But there are also reactions that are not a good sign, and pre-existing conditions where Shiatsu should be discussed first. This article sorts the two out.

Summary/TLDR

Normal after-effects are tiredness, an ache similar to muscle soreness in the treated areas, increased thirst, a feeling of pressure in the head and occasionally an emotional after-effect. All of these are typically over within one to two days. Not normal are stabbing or radiating pain, numbness or tingling in the arms or legs, pronounced bruising, dizziness with vision problems, fever or pain in the calf. People with thrombosis or suspected thrombosis, osteoporosis, acute inflammation, fresh injuries and operations, or cancer, and anyone taking blood-thinning medication, should check with a doctor first. During pregnancy, Shiatsu belongs in experienced hands.

Why there are reactions at all

Shiatsu works with sustained, perpendicular pressure through the thumbs, heels of the hands, elbows and knees, plus stretches and joint rotations. The pressure is not superficial: it deliberately goes into deeper layers of tissue and is held longer than in a classic massage.

That is a mechanical load, and the body reacts to it as it would to unaccustomed exertion. An ache the next day is therefore not a sign that something went wrong, but an expected reaction, especially after the first treatment and in people whose muscles have not been loosened for a long time.

There is also a second effect: an hour of lying still with intense body awareness switches you out of everyday mode. Some people feel a surprisingly strong tiredness afterwards.

What is normal

Reaction Typical duration Assessment
Tiredness, heaviness a few hours to the next day the most common reaction, especially after the first session
Feeling of muscle soreness 1 to 2 days in the areas that were worked intensively
Thirst on the same day drink, that is enough
Mild pressure in the head hours often after work on the neck and shoulders
Emotional after-effect hours to 1 day feeling moved or irritable for no obvious reason
Increased need to urinate on the same evening harmless

These reactions are usually most pronounced the first time and decrease over the following appointments. If you know about them, you will not book the appointment right before an important meeting.

Initial worsening: what is behind it

In Shiatsu practice, a temporary increase in symptoms is often called an "initial worsening" and described as part of the process. That calls for an honest assessment.

That symptoms can briefly increase after an intensive treatment is plausible and known from other manual therapies. The interpretation that this is a necessary step towards improvement, however, is an explanatory model of the method and not a proven mechanism.

In practice, this means: one to two days of increased tension after the session are unremarkable. If it keeps increasing beyond that, "initial worsening" is the wrong explanation. Then the pressure was too high or something else is going on. That needs to be named, not sat out.

Warning signs: when it is no longer normal

With these signs, get medical advice rather than waiting:

  • Stabbing or radiating pain, especially from the spine into the arm or leg
  • Numbness, tingling or weakness in the arms, hands, legs or feet
  • Pain that increases beyond three days instead of fading
  • Pronounced bruising, especially when taking blood-thinning medication
  • Swelling, warmth or pain in the calf: have this checked immediately
  • Dizziness with vision problems, speech problems or a severe headache after work on the neck
  • Fever in the following days

The last three points are the serious ones. They are rare, but they are the reason this list exists. How normal after-effects differ from abnormal ones is also covered in Pain after a massage.

When Shiatsu is not an option without medical advice

Discuss it beforehand if you have:

  • Thrombosis or suspected thrombosis. Pressure on a leg with a clot can be dangerous.
  • Blood-thinning medication. A higher risk of bruising and bleeding into the tissue.
  • Osteoporosis. Deep, sustained pressure is not harmless with reduced bone density.
  • Acute inflammation and fever. The body is already busy.
  • Fresh injuries, operations, open wounds.
  • Cancer. Here the treating doctor decides, not the studio.
  • A slipped disc or acute spinal problems.
  • Pregnancy. Shiatsu is practised during pregnancy, but then belongs in hands with the relevant experience and with a different lying position.

This is not a formality. In these situations, the body reacts differently from what the treatment assumes.

Does this also apply to Shiatsu devices?

Partly. Shiatsu massagers and massage chairs imitate the circular kneading movements mechanically, but work over a wider area and without the targeted depth of thumb pressure. A feeling of muscle soreness after the first longer use is common here too.

The contraindications largely overlap: thrombosis, acute inflammation, fresh injuries and osteoporosis apply in the same way. With devices, there is the added risk that they are easy to use for too long. The built-in brake of a therapist is missing.

How the devices work is explained in How a Shiatsu back massager works and Shiatsu massage chairs. Treatment by a person is described in the Shiatsu guide.

How to reduce the likelihood of after-effects

  • Mention pre-existing conditions and medication at the start, even if you are not asked
  • Say during the treatment if the pressure is too high. That is normal and nobody will comment on it
  • Drink afterwards and do not plan anything strenuous for the rest of the day
  • Choose a shorter session for your first appointment, if offered
  • Do not eat a heavy meal right before

The second point is the most important. Many unpleasant after-effects arise because people put up with pressure that is too high instead of saying so.

Conclusion

For healthy people, Shiatsu is a low-risk method whose most common side effect, tiredness and a feeling of muscle soreness for one to two days, is harmless and passes by itself. It becomes dangerous where pre-existing conditions speak against deep pressure, and where warning signs are dismissed as "initial worsening". If you mention your medical history and speak up during the treatment, you have done the essentials.

Frequently asked questions (FAQs)

Is Shiatsu dangerous?

For healthy people, as a rule, no. Risks arise with certain pre-existing conditions, such as thrombosis, osteoporosis, acute inflammation or cancer, and when taking blood-thinning medication. These should be discussed beforehand.

What side effects does Shiatsu have?

Most commonly tiredness, an ache similar to muscle soreness, thirst and mild pressure in the head, usually for one to two days. Some people also experience an emotional after-effect. All of this normally fades by itself.

How long do the after-effects last?

Usually one to two days, sometimes a little longer after the first appointment. Symptoms that increase beyond three days instead of decreasing are no longer a normal after-effect.

What is an initial worsening with Shiatsu?

This is the term for a brief increase in symptoms after the treatment. That it exists is plausible. That it is a necessary step towards improvement is an explanatory model of the method and not proven. Persistent worsening should not be explained this way.

Can you have Shiatsu during pregnancy?

Shiatsu is practised during pregnancy, but then belongs in hands with the relevant experience, with an adapted lying position and after consulting your doctor or midwife.

Can Shiatsu trigger headaches?

Mild pressure in the head after work on the neck and shoulders is not unusual and usually passes on the same day. A severe headache, especially together with dizziness or vision problems, needs to be checked by a doctor. There is a separate article on migraine.

Why am I so tired after Shiatsu?

An hour of lying still with intense body awareness switches you out of everyday mode, and on top of that there is the mechanical load. Tiredness is one of the most common and most harmless reactions. Better not to book appointments right before important commitments.

Does Shiatsu have to hurt to work?

No. Firm, easily bearable pressure is usual, sharp pain is not. Tensing against the pressure tends to make the treatment ineffective, because the very muscles that are meant to be loosened resist.
