There is a short, clear list: active implants such as pacemakers, acute cervical spine injuries, recent neck surgeries, thrombosis or suspected thrombosis, acute inflammation with fever, unexplained swelling in the neck, and radiating pain in the arm or hand. In these cases, massage is not performed; instead, the condition is clarified. Here are the reasons.

Hard Exclusions

  • Pacemakers, defibrillators, and other active implants. This applies absolutely to devices with electrical stimulation. With purely mechanical shiatsu devices, the situation is different, but proximity to the chest still makes consultation advisable. For more details: Massage chairs and pacemakers.
  • Recent cervical spine injury. After whiplash, the neck should be immobilized and examined, not massaged.
  • Neck or spinal surgery. As long as the wound is not closed and free of irritation, the area remains untouched. What applies afterwards: Massage after surgery.
  • Thrombosis or suspected thrombosis. Pressure on a clot is dangerous.
  • Acute inflammation, fever, neck swelling. Swollen lymph nodes should be examined.
  • Open wounds, eczema, fresh tattoos in the application area.

Warning Signs to Stop Immediately

These also apply during use:

  • Tingling, numbness, or weakness in the arm or hand.
  • Dizziness, nausea, or visual disturbances during use.
  • Pain that radiates to the head.
  • Heart palpitations.

These are not signs of adjustment. They should be medically evaluated before the device is used again.

Cases Requiring Consultation

Not a prohibition, but also not a simple yes:

  • Cervical disc herniation. Depending on the findings, massage can be beneficial or irritating. Comparable classification: Massage chairs with disc herniation.
  • Osteoporosis. Reduced bone density tolerates less pressure, see Massage chairs with osteoporosis.
  • Pregnancy. The neck itself is usually uncritical, but consultation remains advisable.
  • Blood-thinning medications. Bruises form more easily.
  • Migraine. For some, neck work alleviates it, for others, it triggers an attack.

The Most Common Mistake is Not a Prohibition, But the Location

Most post-application complaints do not come from an illness, but from incorrect application. Three areas are delicate: the cervical spine itself, the front of the neck with the carotid artery and larynx, and the base of the skull with full force.

Work should be done on the muscle strands to the left and right of the spine and on the trapezius muscle towards the shoulder. That's where most of the tension from desk work accumulates anyway.

What Helps Instead When the Device Fails

  • Heat without pressure. A cherry pit pillow is often more comfortable for acute irritation than any massage.
  • Movement. Getting up hourly and slowly moving the head works better for "tech neck" than any device.
  • Check screen height. The cause usually lies in the setup of the workplace.

If the complaints are clarified and a device is suitable, the selection is available at neck massagers. Test sitting and trying out are possible at over 40 locations.

In Brief

  • Hard exclusions: active implants, recent injury or surgery, thrombosis, acute inflammation.
  • Stop immediately with tingling in the arm, dizziness, visual disturbances, or heart palpitations.
  • Consult with disc herniation, osteoporosis, blood thinners, and migraine.
  • Never work on the cervical spine itself, never on the front of the neck.
  • For "tech neck," a corrected screen height often works better than any device.
