Why water accumulates in the legs
Blood flows back to the heart through the veins against gravity. Two things help with this: valves in the veins and the calf muscles, which contract with every step. If you stand or sit for a long time, the muscle pump is practically switched off. Fluid then leaks into the tissue, and in the evening, ankles and lower legs are thicker than in the morning.
This is the harmless variant. However, water retention can also come from the heart, kidneys, liver, thyroid, or medication, and in that case, massage only treats the symptom.
What a controlled study found
In 2023, a randomized study with sixty adults with heart failure was published in Holistic Nursing Practice. One half received a ten-minute foot massage on each foot daily for seven days, the other did not. From the fourth session onwards, the differences were statistically significant: swelling decreased, and sleep quality also improved.
Important for classification: This is a single study with sixty participants from a specific patient group. This is an indication, not proof for everyone.
And what argues against it
A Cochrane review from 2015 compiled methods for varicose veins and swollen legs during pregnancy. In it, a study with eighty women compared twenty minutes of daily foot massage with usual care. No significant difference was found in the circumference of the lower leg.

Two other methods performed better in the same review: foot reflexology and twenty minutes of standing in water. Both are also based on small studies with thirty to fifty participants.
The cautious summary is therefore: there are indications that foot massage can help with swollen legs, but they are neither uniform nor strong. Anyone expecting a guarantee will be disappointed.
What makes sense in practice
- Work from bottom to top. From the foot towards the knee, not the other way around. This is the direction the blood should flow anyway.
- Elevate legs. Twenty minutes above heart level often have a more significant effect on evening swelling than any treatment.
- Move. Ten minutes of walking activate the muscle pump, which fails when sitting.
- Compression. The most effective way for recurring swelling, but requires medical adjustment.

When to see a doctor first
Don't wait, but have it checked if: unilateral swelling, especially with warmth, redness or pain in the calf; sudden swelling; swelling along with shortness of breath; swelling that doesn't subside in the morning. A unilaterally swollen, painful calf can be thrombosis, and then massage is dangerous instead of helpful.
